Patzers are enemies consisting of a floating brain surrounded by a large sawblade. The sawblade is completely invulnerable and will block Isaac's tears; the brain must be killed to destroy it.
Both parts of Patzer will follow a periodic pattern of movement; The brain will idle in place for a moment before it begins moving, floating to a position close to Isaac and leaving the sawblade behind while doing so. During this period the brain is vulnerable to being hit by Isaac's tears until the sawblade begins to follow the brain's movements shortly afterwards. The brain will start moving again as soon as the sawblade catches up to its position.

- Patzer was one of the first teased enemies for The Reheated Update.
- The term patzer refers to someone who is an amateur at chess.
- Patzer's shell is canonically a woman and thus charmed by
Sapphic Sapphire, 
Homoerotic Ruby and 
Gay Garnet. Patzer's brain, however, is not, and will typically die to its own shell when it attempts to move away.
