Apega
Apegas are enemies encountered exclusively in Gehenna that make short hops in the cardinal directions towards Isaac. They are invulnerable while their doors are closed and will deflect tears that hit them, similar to Rubber Cement's effect. After hopping for a awhile, an Apega stops in place to swing open its doors, becoming vulnerable, and fire out a ring of 10 spike-shaped projectiles which deal a whole heart of damage. These projectiles will return back towards Apega after a moment, after which the Apega closes its doors and resumes hopping towards Isaac.
If Isaac is close to an Apega, it will hop 1-3 times between attacks, and 3-6 times if Isaac further away. Although, if Isaac enters its range while hopping, it can immediately stop and perform its attack.

- Although always facing the screen, Apegas are vulnerable from all sides when open.
Trivia
- Apega's design is inspired by the Iron Maiden, a potentially fictive medieval torture device consisting of an iron coffin lined with spikes on the interior.
- Furthermore, Apega's name comes from the Apega of Nabis, a similar torture device from ancient Greece.
- Apega's behavior is a reference to one of
The Visage's attacks as they appeared Antibirth, where in its third phase the heart would shoot out a ring of projectiles and then suck it back in ― an attack which was inexplicably absent in The Binding of Isaac: Repentance.
- Their behavior is also a reference to the Lead Maiden enemies from the game Enter the Gungeon.
