Death's Heads
Death's Heads are skeletal enemies that bounce around the room diagonally and are generally invulnerable. They die after all other enemies in the room have been killed.

Lil' Jon
Lil' Jons are giant Death's Heads that travel slower than their counterparts and die upon the room's clear as usual. Unlike normal Death's Heads, they collide physically with Isaac and can block his movement.
Elite Jon
Elite Jon is a unique variant of Lil' Jon that only shows up in The Real Jon Challenge. The Elite Jon behaves the same as Lil' Jons, but will not disappear when the room is cleared, and will follow Isaac through every room. It will spawn in the center of every room, or as close to the center as possible.
Notes
- Elite Jon can be killed by effects like
Chaos Card or
Euthanasia, but will still reappear upon entering another room.
Dope's Head
Dope's Heads are a variant of Death's Heads found in Dank Depths. They move slightly slower than their counterpart, but will gain a burst of speed when bouncing off of a wall or another Dope's Head.
Astroskull
Astroskulls are a variant of Death's Heads found in Utero. Astroskulls will always be accompanied by smaller skulls, referred to as Lil' Junkies, which will orbit around them in a slow pulsating pattern and can rotate either clockwise or counter-clockwise. Lil' Junkies will die along with their respective Astroskull, however Lil' Junkies themselves are completely immune to effects such as Euthanasia.

- The amount of Lil' Junkies an Astroskull spawns with, as well as its rotation direction, is determined by the room layout.
Cherubskull
Cherubskulls are a variant of Death's Heads found in Cathedral. Cherubskulls attempt to bounce around the room diagonally, while being chained to a skeletal arm which restricts its movement. Each time it fully extends the length of its chain, it will snap back in the opposite direction while also causing the skeletal arm to wiggle a bit in place. They will bounce back and forth in this pattern for a short moment, before the arm loses traction and will slide freely for a random brief amount of time, allowing the Cherubskull to travel further for the duration. While Cherubskulls can fly, their skeletal posts will get stuck on obstacles and pits, further limiting their movement.
A Cherubskull's skeletal arm can also deal contact damage, though the chain can be passed through safely.
Killing a Cherubskull with effects such as Euthanasia will destroy the arm its chained to. However, destroying a Cherubskull's arm will release the skull, causing it to behave like a regular Death's Head.

- The length of a Cherubskull's chain is variable and determined by the room layout.
Blazer
Blazers are a variant of Death's Heads found in Sheol that resemble The Lamb. They behave similarly to their counterparts, however, if Isaac enters their range, their eyes will flash and a sound cue will play. The Blazer will then quickly dash towards Isaac while leaving a short trail of blue fire that dissipates quickly.
Trivia
- The behavior of the Cherubskull is inspired by an interaction between Death's Heads and the item "Derelict Anchor" from the Retribution MOD.
- Astroskull and Lil' Junky are referred to as "Craterhead" and "Craterhead Jr." respectively in the MOD's image files.
- The Blazer coincidentally bears a resemblance to the Lost Soul from the video game series Doom, with both being large, flaming horned skulls that charge at the player.
