Splattercap
Splattercaps are large stationary mushrooms are always accompanied by a Cordy. If a Cordy is in the room, they will jump on top of Splattercaps periodically, releasing a large amount of short-ranged red projectiles in all directions with each landing. The projectiles they release deal a whole heart of damage.
If Splattercaps are the only remaining enemies in the room, they will explode into a damaging fart.
Cordy
Cordies are fungal Leaper-like enemies that are generally spawned alongside Splattercaps but can sometimes be encountered on their own. If there are Splattercaps in the room, they will jump high into the air and land on a Splattercap to trigger its attack. They will prefer to switch to a different Splattercap with each jump if possible. While on top of or jumping between Splattercaps they cannot be damaged or killed.
However, they can still be killed as long as they are grounded meaning they can be killed when they first spawn in a room. Additionally, if a Cordy is on top of, or about to land on a Splattercap, killing that Splattercap will cause it briefly land on the ground and become vulnerable.
If no Splattercaps exist or once they are all killed, Cordies will jump aggressively after Isaac, always directly landing on his position at the time of jump.

- Splattercap's appearance is based on the Bleeding Tooth Fungus, known for its trypophobia-inducing appearance.
- Splattercap is a returning enemy from the Devil's Harvest MOD, originally named Fatshroom.
- Splattercaps and Cordies have unique appearances when encountered on the Grotto floor if the Fall from Grace MOD is installed.
