Mr. Maws
Mr. Maws are characterized by having a body and head which can be separated, and they will often use their heads as a weapon.

Mr. Horf
Mr. Horfs pursue Isaac, lobbing their head towards him as they get close by. The head will become a 
Horf upon landing, while the body continues on as either a 
Pacer or 
Gusher.
If killed before they can lob their head, they will turn into a Pacer or Gusher.
Mr. Red Horf
Mr. Red Horfs are the 
Red Horf counterpart to Mr. Horfs. They are slightly faster and throw a Red Horf instead. The Red Horf spawned will be fully swollen, and will quickly explode if Isaac lingers near it.
Mr. Sub Horf
Mr. Sub Horfs behave nearly identically to their counterparts; however, instead of lobbing their head, they will spin it towards Isaac instead. This causes the spawned 
Sub Horf to fire out projectiles immediately upon being released, and due to not being lobbed, it cannot travel over obstacles.
Mr. Gurgle
Mr. Gurgles behave similarly to their counterparts; however, their heads will detonate on impact, destroying it and creating a large pool of green creep where it lands. Their remaining body will always become a 
Splasher.
Notes
- The explosion from the head deals 20 damage to all enemies caught in it and can also kill its own Splasher.
- The head can be killed before it hits the ground and will not explode if killed.
Mr. Bones
Mr. Bones are skeletal Mr. Maws that resemble 
Bonies. They roam aimlessly around the room instead of chasing Isaac. When Mr. Bones is near Isaac, it will launch its head towards him, trying to bite him. If the head is killed first, Mr. Bones turns into a pale white-colored Gusher, not to be confused with an actual 
Pale Gusher.
Notes
- Mr. Bones have a random chance to replace Bonies, which is intended to catch Isaac off-guard.
- The only ways to tell Mr. Bones apart from a normal Bony is the jitter its head makes while it walks, and the blood partially visible on its body.
Mr. Gob
Mr. Gobs behave similarly to 
Mr. Maw, launching their heads towards Isaac when close. However, after its neck is fully extended, Mr. Gob will remain stationary for an extended period, while the neck cord releases tar projectiles perpendicularly. On death, they will turn into either a 
Gob or a 
Black Globin's Body, depending on if the head or the body was killed first.

- Mr. Sub Horf and Mr. Gurgle were added in the V2.8 update.
- Mr. Sub Horf was unused prior to the update.
- Mr. Sub Horfs have a unique appearance when encountered on the Boiler floor if the Fall from Grace MOD is installed.
- Mr. Bones have a unique appearance when encountered on the Tomb floor if the Revelations MOD is installed.
- Mr. Bones have a unique appearance when encountered replacing Bonies on the Future floor if the THE FUTURE MOD is installed.
