Fatties
Fatties feature a larger-than-average health pool but slow movement speed while chasing Isaac. They spontaneously fart as their main attack.

Big Smoke
Big Smoke is an ashy Fatty variant that behaves very similarly to a standard Fatty. However, it will leave behind a ring of gunpowder when farting and upon death.
Tubby
Tubby are drowned Fatties that chase Isaac like usual. Occasionally it will either fart and spawn several 
Bubbles in random directions, or stomp its feet in place repeatedly, scattering short-ranged projectiles around itself for a period. It will also perform the stomping attack when Isaac gets too close.
Squidge
Squidge are a dank variant of Fatties that scatter three large projectiles in a triangle pattern when they fart. These projectiles spawn 
Tar Bubbles where they land.
Squidges will also leave a small puddle of slowing black creep upon death.
Globwad
Globwads are large 
Globins in the form of a Fatty. They behave similarly to regular Fatties, but when farting, create a large puddle of red creep and shoot out a short-ranged disorganized ring of projectiles that hover in place for a moment. When killed, a Globwad will leave behind a large pile of goo which scoots away from Isaac while leaving a trail of red creep. If left alone for long enough, the goo will reform into a Globwad with decreased health.
If killed in goo form, it creates a large puddle of damaging red creep, shoots projectiles in the cardinal directions, and spawns 2 or 3 Globins in their goo form, which reform soon after.
Notes
- When turned into Globwad goo, the goo will be at half of the Globwad's maximum HP. The Globwad's HP will also be reset to half of their max HP again when reforming from their goo form.
- Globins created from a Globwad's final death will spawn at half of their maximum HP.
Mouthful
Mouthfuls are scarred variants of a Fatties. They fart more frequently while chasing Isaac, with each fart spawning 1-3 
Morsels.
Upon death, Mouthful will spawn another 4-5 Morsels.
Morsels spawned by the Mouthful have unique appearance.

- Big Smoke is named after the eponymous Big Smoke from the game Grand Theft Auto: San Andreas.
- Squidge's design looks very similar to the co-op Bubbles Baby skin.
- Squidge was originally called "Dank Fatty".
- Mouthful was originally called "Mama Fatty".
- Globwad was originally from the unreleased Morbus MOD.
