Forgotten Fetus
Forgotten Fetuses are large fetal enemies that crawl in quick bursts towards Isaac. After each crawl they will stand still for a moment or attack before moving again. When attacking, Forgotten Fetuses will fire a single large projectile toward Isaac that will trail a line of red creep behind it and create a larger puddle of creep where it lands.
Forgotten Fetuses always spawn with a Lil Al attached to them which will be dragged along behind them. If the Lil Al is killed before the Forgotten Fetus, or if the cord takes 20 damage and is destroyed, the Forgotten Fetus will enrage and increase its chasing speed. When enraged it will also attack every time it stops, with the projectile now releasing a small ring of 8 arcing projectiles wherever it lands as well as another secondary burst of projectiles toward Isaac.
A Forgotten Fetus can spawn with multiple Lil Als attached to it and will only enrage after all of them have been killed or detached.

- The length of a Forgotten Fetus' cord as well as how many Lil Als it spawns with is determined by the room layout.
Lil Al
Lil Al always spawns attached to a Forgotten Fetus. While attached, Lil Al will constantly trail a thin line of long-lasting red creep when it is dragged along. If the Forgotten Fetus is killed or the cord is destroyed, Lil Al will begin to cry, firing many arcing projectiles around it that leave small puddles of red creep wherever they land.

- Forgotten Fetus and Lil Al were added in The Spookaween Update.
- In the files, a variant of Lil Al with a single hair can be found, though it appears to be unused.
- Forgotten Fetus' design was inspired by the Baby monster from the game, Resident Evil Village.
