Madclaws are crab-like enemies that dive underwater immediately after spawning. While underwater, Madclaws are only visible by their shadow and will perform one of two attacks:
- The shadow moves to an edge of the room. The Madclaw will then stick its claw from beneath the water and charge at Isaac, leaving a wake of projectiles from both sides. This attack will only end once the claw hits a wall or obstacle.
- Madclaw's shadow chases Isaac, and it attempts to snap at Isaac with its claw 1-3 times from underneath him. This attack deals a full heart of damage.
Afterwards, the Madclaw will then resurface from a random location within the room and begin to chase Isaac for a period. It will then dive down to perform its other attack, before resurfacing again to chase for another period, and then restarting the cycle.

- If there are any
Bubble Blowing Double Babies in the room, a Madclaw's snap attack will target them first and separate them.
