D3 is a passive item added in The Reheated Update.
Effects
- Spawns a large blue dice familiar in the shape of a triangular prism that orbits around Isaac in a sine wave pattern.
- Any of Isaac's tears that pass through the D3 orbital will be rerolled, gaining a slew of random effects:
- Applies a random color and appearance to the tear (which is merely cosmetic).
- Modifies the tear's damage by a random amount, between 50% to 150% of its damage.
- Modifies the tear's shot speed by a random amount, between 10% to 150% of its shot speed.
- Applies a random amount of tear effects from the vanilla game and the Fiend Folio MOD, which are listed below.

- May drop Pennies when a tear hits an enemy (as opposed to only when killing them), similarly to tears shot by Bumbo (Level 3)
- Teleport enemies to a random location in the room (cannot affect bosses)
- Transform enemies into Poop (similarly to E. Coli)
- If dealing overkill damage, tear will continue traveling with leftover damage (similarly to Polyphemus)
Notes
- The D3 orbital cannot reroll enemy projectiles/attacks, and does not affect enemies on contact.
- A tear that had already been rerolled by the D3 cannot be rerolled again.
- The D3 can apply up to 10 different tear effects to a tear, though the amount of effects a tear can get is linearly chance-based — the chance for a tear gaining 1 effect after being rerolled are 10/55, 2 effects are 9/55, etc., all the way to 1/55 chance for a tear to gain 10 different tear effects after being rerolled.
- The appearance of a tear when it is rerolled usually may not correlate to the tear effect it can apply.
- Bombs placed by Isaac can be rerolled, gaining the same possible random effects, as well as gaining some possible bomb upgrade effects like those from Nancy Bombs.
- Brimstone beams and Technology lasers shot by Isaac that pass through the D3 orbital will each create a new laser that, in addition to gaining whatever tear effects are possible, will also have one of these properties chosen at random:
- Technology laser.
- Revelation beam, that can be of random duration, range and width.
- Brimstone beam that can be of random duration, range and width, and which themselves can have additional appearances:
- Brimstone beam that has a Technology laser coiled around it.
- The Beast's colossal Brimstone beam.
- Sapphic Sapphire Brimstone beam, with the appearance of the Lesbian or Trans flag.
- Pride's scribble-y wave-like laser.
- Play a different sound from that of other laser types (merely cosmetic effect).
Interactions
- BFFS!: Familiar modifiers will have no effect on the D3.
- Spindown Dice: Rerolls into Strange Red Object.
Trivia
- The D3 is a returning Item from The Devil's Harvest MOD from Afterbirth+.
- In The Devil's Harvest, it was an Active Item that, when used, rerolled all of Isaac's tears on screen.
- The D3 is the only dice item in Fiend Folio that is a Passive Item as opposed to an Active.
- The D3 incidentally shares many of its possible tear effects with Fruit Cake
- The D3 is very wacky.
