Fetal Fiend is a passive item added in The Reheated Update.

Effects

• +1.4 damage.
• Replaces all Soul Hearts and Black Hearts in Isaac's health bar with Immoral Hearts.
• All of Isaac's Red Hearts in his health bar are converted to Immoral Hearts at a 2:1 ratio, leaving Isaac with empty Red Heart containers.
  • Both full and half-filled Red Heart containers in the health bar will be emptied and equally converted to a Half Immoral Heart.
  • Having empty heart containers will not typically kill Isaac due to Immoral Hearts always being granted, so it won't leave Isaac without any health remaining.

Interactions

• Alabaster Box: Taking Fetal Fiend while holding Alabaster Box will cause all Immoral Hearts generated from converting Red Hearts to be converted to Alabaster Box charges. However, this will still empty all Red Heart containers, and if Isaac had no Soul Hearts or Black Hearts in his health bar prior to picking up Fetal Fiend, this will kill him.
• The Forgotten: If picked up as The Forgotten, empties all Bone Hearts and grants the respective amount of Immoral Hearts to The Soul, without converting The Soul's Hearts. If picked up while switched to The Soul, all Soul Hearts and Black Hearts are instead converted to Immoral Hearts without emptying The Forgotten's Bone Hearts.
• Bethany: Leaves Bethany with half of a Red Heart remaining. Bethany obtains no Immoral Hearts and instead gains 1 soul charge for every Red Heart converted.
• Keeper / Tainted Keeper: Will not affect any Coin Hearts.
