• Fiend's Third Leg is not in any item pool and is set to be hidden from appearing in any normal scenario.

Trivia

• Fiend's Third Leg is a joke item based off of an image, created by a user named melon (a renown developer of mods such as Revelations and Bertran), of a plushy resembling a purple imp with a strange protrusion between its legs that resembles a "third leg". The image became coined as Fiend's "massive MOD".
