Fiend Mix is an active item added in The Reheated Update.

Effects

• Consumes half of Isaac's health on use, transforming each full heart into a Minion and each half heart into a Hollow Minion.
  • These Minions act like those created from Immoral Hearts, but will persist between rooms without enemies. Minions will turn back into their respective Heart pickup upon clearing the next room.
  • Fiend Mix will not consume any hearts if it would kill Isaac.

Notes

• With the exception of Minions created from Black Hearts, all minions of each heart type deal the same amount of damage (20% of Isaac's damage) and have the same chance to land a critical hit. Additionally, Hollow Minions will always transform back into a Half Heart pickup upon clearing a room.
  • Minions created from Black Hearts will deal 40% of Isaac's damage instead of 20%, and will have a higher chance to land a critical hit. Hollow Minions created from Half Black Hearts will not turn back into a Half Black Heart when the room is cleared, instead disappearing into a puff of smoke.
• Gold Hearts and Rotten Hearts will always turn into regular Minions while Eternal Hearts will always turn into Hollow Minions.
• Minions created from Morbid Hearts, instead of turning back into Morbid Hearts on room completion, will instead act and appear as Red Heart Minions and turn into Red Hearts.
  • This means if an entire Morbid Heart is converted, the remaining 3 Half Red Hearts worth of pickups will exceed the original Heart Container capacity with an extra Half Red Heart leftover.
  • It is unsure whether this is intentional or an oversight.

Interactions

• Book of Virtues: Creates a unique Fiend wisp. When destroyed, these wisps spawn an Evil Hollow Minion in the current room. Evil Hollow Minions will not drop a Black Heart when they disappear on room clear.
• Candy Heart / Soul Locket: Minions created by Fiend Mix turn back into heart pickups which may activate the effects of these items when collected. This will essentially allow Isaac to accumulate massive amounts of stat upgrades, for as long as there are rooms to clear and Isaac keeps the correct kind of hearts in his health bar.
• Immaculate Conception: Minions created by Fiend Mix turn back into heart pickups which will count towards this item's activation when collected.

Trivia

• Fiend Mix is a reference to the show Making Fiends, notably the box labeled "Fiend Mix" that can be found in the show's intro.
• Minions created from Morbid Hearts have sprites in the files, however these are currently unused.
