Lil Fiend is a passive item added in the initial release of Fiend Folio.

Effects

• Spawns a fiendish familiar that bounces around the room and blocks enemy projectiles.
  • The speed at which Lil Fiend moves changes with distance to Isaac, slowing down when Isaac stays close and speeding up when farther away.
• Each time Lil Fiend blocks a projectile or makes contact with an enemy, there is a 20% chance for Lil Fiend to spawn an Evil Hollow Minion.
  • Evil Hollow Minions do not drop Black Hearts when the room is cleared, instead disappearing with a puff of smoke.
• Lil Fiend contributes towards the Conjoined transformation.

Interactions

• BFFS!: Increases Lil Fiend's size and increases the chance for Lil Fiend to spawn Evil Hollow Minions to 33%. The Minions themselves are unaffected.
• Box of Friends: Lil Fiend can be copied to generate a second familiar for the duration of the current room.
• Sacrificial Altar: Can be sacrificed as normal.
• King Baby / Duct Tape / Friendship Necklace / RC Remote: Has no effect on Lil Fiend.
