Crazy Jackpot is a passive item added in The Reheated Update.

Effects

• When Isaac takes damage, slot machine reels will appear above Isaac's head that activate an effect depending on the outcome.
  • If the symbols of the three reels do not match, then nothing will happen.
  • If the symbols all match, then a positive effect will activate depending on the symbols:
    • Gem: Spawns 1-2 coins.
    • Cherry: Spawns a heart that disappears after 1 second.
    • Bell: Creates a red pulse around Isaac that deals 25× his damage to all enemies in the room and pushing them away.
    • Horseshoe: Grants Isaac -20% tear delay, +20% damage or +3 luck until Isaac enters a new floor. These stats can stack when activating multiple times.
    • Lemon: Activates the Free Lemonade item effect, creating a large yellow creep puddle under Isaac that deals 8 damage per tick (or 24 damage per second). The creep puddle disappears after ~19 seconds.
    • Seven: Rarest of the outcomes; activates all of the above effects at once.

Notes

• The chance to match any 3 symbols and gain an effect is affected by Isaac's luck, being at an approximately 30% chance at 0 luck, and reaching 100% chance at 10 luck.
• Coins and Hearts spawned by Crazy Jackpot are random and follow normal random drop rules, such as potentially spawning Nickels, Soul Hearts, etc.
• Crazy Jackpot's slot machine reels take roughly 2.5 seconds to complete their spinning and activate their effects. Taking damage again while the reels are still spinning will queue the additional spins until each one has concluded.
• The spinning of Crazy Jackpot's slot machine reels will persist between rooms, meaning the effect can be activated in a prior room and carried over to another, such as through Curse Room doors.

• Crazy Jackpot is a reference to the song Hommarju feat. Mayumi Morinaga - Crazy Jackpot, used in the game REFLEC BEAT VOLZZA and beatmania IIDX 28 BISTROVER.
