Horncob is a passive item added in The Reheated Update.

Effects

• Isaac shoots pill tears that, upon killing an enemy, has a 25% chance to activate a random pill effect.
  • These pills can be any random pill from the entire game, and may not be in the pill rotation of the current run.
  • This chance increases by 5% for every point of luck, reaching a 100% chance at 15 luck.

Interactions

• Abyss: Absorbing Horncob spawns a grey Abyss Locust that looks like a pill. This locust has a chance to activate a random pill effect when killing an enemy.

Trivia

• Horncob is the only item that is exclusive to the Dire Chest item pool in Fiend Folio: Reheated.
• In one of the teasers on the official Fiend Folio Twitter account leading up to Reheated's release, Horncob was seen being held as a pocket active by Fiend, along with its unique costume.
  • This was intentionally made to act as a red herring to appear as a "Tainted" version of Fiend, in order to obscure The Bastard's true appearance.
  • Additionally, in-game when Horncob is picked up as Fiend, white text will appear above Fiend that reads "The Unscrupulous", the supposed title of the "fake" Tainted Fiend.
