Shard of China		Put yourself back together	Overhealing with Red Hearts charges a special health bar. Filling this Health bar accumulates a damage boost, up to +1.5 damage for each Heart filled. Smelts itself once the bar is completely filled.
Bifurcated Stars		Rush the boss, reap rewards	Reveals the Boss Room on the map. The Boss Room exit door is replaced with a door to the Treasure Room after the room is cleared.
Fortune Worm		Blah Blah!	+0.2 shot speed. +2 luck. Luck-affected chance to fire fortune cookies that deal 1.05× damage and will display a random fortune on screen when hitting an enemy.
Fool's Gold		Chance for more money?	Cursed Pennies can drop on room clear. Cursed Pennies have a higher chance to replace Pennies.
Extra Vessel		HP up	Stores an extra Red Heart, which acts as an extra Red Heart Container. Does not count against Devil Deal chance if damaged while filled.
Chili Powder		The ol' spicy keychain!	Key pickups are turned into Spicy Keys. Spicy Keys grant an extra key upon being picked up, but deal damage to the player.
GMO Penny		Wealth of preservatives	Chance to take a random pill when picking up a coin. Scales with luck.
Molten Penny		Wealth of labor	2% chance to activate the Smelter effect when picking up a Penny. Scales with luck.
Bloody Sandpaper		STOP!	Isaac no longer has movement momentum.
The Right Hand		The right-hand path reaps light rewards	Chests have a 1/3 chance of becoming Eternal Chests.
Red Ribbon		Twice as eternal!	Eternal Hearts are converted to Full Eternal Hearts, which immediately grant a Red Heart container on pickup.
Flea of Meltdown		I bring Meltdown	Creates an Orange Flea when entering a new uncleared room. Orange Fleas will burn enemies on hit.
Flea of Deluge		I bring Deluge	Creates a Cyan Flea when entering a new uncleared room. Cyan Fleas will create a ring of 8 tears that deal Isaac's damage on hit.
Flea of Pollution		I bring Pollution	Creates a Purple Flea when entering a new uncleared room. Purple Fleas will leave behind a slowing black creep puddle on hit.
Flea of Propaganda		I bring Propaganda	Creates a Beige Flea when entering a new uncleared room. Beige Fleas will charm enemies on hit.
Fuzzy Penny		Wealth of fuzz	Spawns a Blue Skuzz every time a Coin is picked up.
Fortune Grub		The slot machines whisper	+1 luck. Beggars and Slot Machines will display a fortune when interacted with, or alongside their usual payouts.
Chunk of Tar		Sludge friends	Summons 1-3 friendly Blots on room entry.
Yin-Yang Orb		Homing amulet	Luck-based chance to fire a tear with 1.25× damage and aggressive homing.
Swallowed M90		PCHEW!	When Isaac takes damage, pulls out an M90, causing the next shot to be a fast, piercing bullet that does 3.5× Isaac's damage with self-knockback.
Trinity Worm		Vwoo vwoo vwoo!	+0.4 tears. +12.5 range. Isaac's tears become spectral and travel in the shape of a triquetra, or 'Holy Trinity'.
Jigsaw Puzzle Box		There's always a piece missing...	Only once, drops a Puzzle Piece on floor start. Raises the chance for Puzzle Pieces to drop. Once a Puzzle Fortune is completed, this trinket destroys itself.
IOU		I'll give it back soon	Increases the chance of trinkets dropping after clearing a room. If held at the end of the run, spawns a random trinket at the start of the next run.
Autopsy Kit		Organ harvesting	Bombed Shopkeepers will drop Red Hearts or Bone Hearts instead of Coins. Heart chances increase with the quality of coins replaced.
Spire Growth		Power in the cards	Using a Card with a number will grant a fading damage up that scales with the number.
Flea Circus		Infested	Chance to spawn a random Flea when an enemy is killed.
Jevilstail		I can do anything!	Grants a rotating list of random, wacky effects for each room, which can be either helpful, harmful, or neither.
Dealmakers		Specil deals	Randomizes Shop prices. Some Shop items will have constantly shifting prices.
Solemn Vow		Do no harm	Allows the player to see enemy health.
Massive Amethyst		Crystallised keepers	Shopkeepers in Secret Rooms are replaced by Amethyst clusters. Amethyst clusters drop Runes when destroyed.
Lost Flower Crown		Forgotten memories	Once a floor, activates the Eraser effect on a random enemy, removing that enemy from the run.
Sharp Penny		Wealth of pain	Chance to activate the Dull Razor effect when picking up Coins.
Angry Faic		Heartier bosses	Floor bosses and mini-bosses have increased HP, but also drop more Heart pickups on death.
Eternal Car Battery		???	Active items are triggered 4-6 times, but have a 1/2 chance to disappear on use.
Pocket Dice		Lucky! New rolls each floor	Drops 2 random Glass Dice and a Penny at the start of each floor.
Locked Shackle		All stats up, missing its key	All stats up. Can't be dropped. Picking up Keys has a 25% chance to destroy the trinket and use a Key.
Petrified Gel		It tastes lucky?	Poops have a chance to be converted into Shampoo. Shampoo will no longer spawn Drops and will drop better rewards.
Hatred		Double tap distilled malice	Shooting familiar that moves with the player and can be fired as a piercing projectile by double tapping. Takes a few seconds to reform after hitting an obstacle. Will stand still if the drop button is held.
Frog Puppet		Greener on the other side	If Isaac is about to take fatal damage: Turn into Slippy. Adds Golden Frog Head as a pocket active item. Health bar is converted into 3 filled Red Heart containers.
Wackey		Wackier key drops	Key drops have an equal chance to become any of the Key types.
Heartache		Hurts on the inside	All damage taken gives a Broken Heart instead.
Grabber's Enemy		Caught you!	Flying Grabber hand familiars spawn that drag Isaac, pickups, and enemies around.
The Middle Hand		The middle-hand path reaps dire rewards	50% chance for any Chest or Locked Chest to be replaced with a Dire Chest.
Rainbow Bean		Intimate farts	When standing near enemies, the player has a chance of expelling a random Bean fart.
Faulty Fuse		Tastes like burnt copper	Increased chance for Copper Bombs to spawn.
Cursed Urn		Fragile revival	Strong tears up. If Isaac is about to take fatal damage: Turn into China. Health bar is converted into China's health bar. Shatters if dropped, turning into Shattered Cursed Urn and losing its ability to revive.
Conjoined Card		Miniature emperor	An additional Mini-Boss Room will generate on every floor. Excludes Blue Womb, The Void, and Home.
Energy Searcher		Draw energy	Increased chance for Energy Cards to drop on clearing a room. Energy Cards have a 33% chance of spawning another one when used.
Broken Record		Stuck on repeat	Chance for newly-spawned passive collectibles to be replaced with one Isaac already has. Will not provide Isaac with completely useless duplicates.
Fuzzy Pickle		Reference up!	Grants +0.1 damage for each reference item / trinket Isaac has. Active items grant +0.5 damage instead. Loose references only grant half of the corresponding damage.
Fushigi		Make it float!	Luck-affected chance to fire a metal tear with the Eye of the Occult effect with increased range and double damage.
Nesting Doll		Surprises inside	When blowing up a Beggar, has an 80% chance to spawn a different random Beggar in its place.
Egg Penny		Wealth of progeny	Chance to spawn a Fragile Bobby on picking up a Coin.
Leftovers		Save it for later	Overhealed Health gained from Heart pickups will be left on the ground for later use.
Dud's Flower		Dud bombs	All Bombs created by Isaac will become Copper Bombs.
Bomb Token		Explosives up	Increases the damage dealt by placed Bombs by 100. Grants a 1.5× damage multiplier to Dr. Fetus Bombs (instead of the flat damage up).
Top Rock		All stats up! It's all downhill from here.	All stats up. Prevents stats from being increased while held.
