Cards are pickups with a wide variety of effects when used. They take up the same slot as pills, runes and objects when picked up. Their effects can be mimicked by using Blank Card.

Fiend Folio adds 60 new cards of various kinds, either based on tarot cards, playing cards, Pokémon cards or other card games!

Minor Arcana Cards

The Minor Arcana are a set of cards based on the the cartomantic tarot decks.

Ace of Swords				"Convert all"	Turns all pickups and non-boss enemies in the room into Blue Flies, Blue Spiders, or Blue Skuzzes.
Ace of Cups				"Convert all"	Turns all pickups and non-boss enemies in the room into pills.
Two of Wands				"Item multiplier"	Doubles the current charge of active items held by the user. Adds one charge to active items that currently have no charge. Can be used to overcharge active items.
Two of Pentacles				"Item multiplier"	Spawns copies of all trinkets currently held by the user. Spawns a random trinket if no trinkets are currently held. Does not spawn duplicates of gulped/smelted trinkets.
Two of Swords				"Item multiplier"	Doubles all of Isaac's 'temporary' familiars. This includes Dip familiars, Blue Flies, Blue Spiders, Blue Skuzzes, Fragile Bobbies, Minisaacs, and Fairy Flies. Also activates the Box of Friends effect, duplicating all of the user's permanent familiars for the room.
Two of Cups				"Item multiplier"	Spawns a copy of every pill in the room and in Isaac's pocket slot. If there are no pills to duplicate, spawns 2 random pills instead.
Three of Wands				"Item smorgasbord"	Spawns 3 random batteries that disappear after 3 seconds. These batteries have an equal chance to be of any variant.
Three of Pentacles				"Item smorgasbord"	Spawns 3 random trinkets, which includes rock trinkets. These trinkets have a 50% chance of being golden.
Three of Swords				"Item smorgasbord"	Spawns 3 random 'temporary' familiars. This includes Dip familiars, Blue Flies, Blue Spiders, Blue Skuzzes, Fragile Bobbies, Minisaacs, and Fairy Flies.
Three of Cups				"Item smorgasbord"	Spawns 3 random pills. These have a 50% chance of being horse pills.
King of Wands				"Battery upgrade"	Upgrades all batteries in the room to their next tier. Potato Batteries → Micro Batteries Micro Batteries and Cursed Batteries → Lil' Batteries Lil' Batteries, Virtuous Batteries, and Firework Batteries → Mega Batteries Mega Batteries → Golden Batteries Storage Batteries and Corroded Batteries → Lil' Batteries
King of Pentacles				"Trinket upgrade"	Converts all trinkets in the room and held by the user into golden trinkets. Has no effect on gulped/smelted trinkets.
King of Swords				"Familiar upgrade"	Grants the effects of BFFS! for the duration of the current floor.
King of Cups				"Pill upgrade"	Converts all pills in the room and held by the user into horse pills.

Playing Cards

Playing Cards are generally based on suit cards. In The Binding of Isaac, most of them are centered around the 4 major consumables and their variants: Hearts, Coins, Keys, and Bombs.

Jack of Clubs				"Destruction at what cost?"	Spawns 2-3 Copper Bombs. Converts all bombs in the room into Copper Bombs.
Jack of Diamonds				"Wealth at what cost?"	Spawns 2-4 Cursed Pennies. Converts all coins in the room into Cursed Pennies.
Jack of Spades				"Keys at what cost?"	Spawns 1-2 Spicy Keys. Converts all keys in the room into Spicy Keys.
Jack of Hearts				"Health at what cost?"	Converts all of Isaac's Soul Hearts and Black Hearts in the health bar into Immoral Hearts. Grants 1 Immoral Heart if there are no Soul Hearts or Black Hearts to replace in the health bar.
Queen of Clubs				"ó"	Grants the effect of Dr. Fetus for the current room.
Queen of Diamonds				"Obstacles = [$]"	Converts each obstacle in the room into a Penny that disappears after 5 seconds. Tinted Rocks and Fool's Gold Rocks are converted into Nickels instead.
Queen of Spades				"0-^^"	Drops 1-10 Keys.
King of Clubs				"BOOM BABY!"	Drops a Giga Bomb.
King of Diamonds				"Gild all before you"	Turns the entire room and all enemies golden, preventing them from moving and causing them to drop Pennies on death, for 8 seconds. Converts up to ~25% of the Rocks in the room into Fool's Gold Rocks.
King of Spades				"Unlock your wrath"	+1 key. For the duration of the current room, tears fired by Isaac will turn into Sharp Key projectiles. Sharp Key tears will cost 1 key to fire, but will drop a key where they land. If Isaac has no keys, then Isaac will shoot normal tears.
Misprinted Joker				"Twist into good or evil"	Has a chance to either reroll each item in the room into those from the Devil Room item pool and Angel Room item pool, or convert them into a Soul Heart, Black Heart, Eternal Chest, or Dire Chest.	Same as Joker
Misprinted Jack of Clubs				"Destruction at what cost?"	Spawns a Golden Troll Bomb. Converts all enemies in the room into Troll Bombs.	Same as Jack of Clubs
Misprinted 2 of Clubs				"= [$]Item multip"	Doubles the number of Isaac's held Bombs. Converts every Bomb Isaac has into Copper Bombs.	Same as 2 of Clubs
Thirteen of Stars				"This is stupid!"	Spawns a glitched TMTRAINER item, or a 0.11% chance for Perfectly Generic Object.
King of Clubs?				"Oh my god we're doomed!"	Isaac creates a Giga Bomb explosion, which kills him instantly. This card can not be obtained normally without the use of the debug console.

Magic: The Gathering Cards

Magic: The Gathering Cards are cards based on the Magic: The Gathering trading card game.

+3 Fireballs?				"Loaded with malice"	Transforms Isaac into a Malice fireball that bounces off of walls and obstacles, up to 6 times. Enemies killed by the fireball have a chance to drop Half Black Hearts.

Energy Cards

Energy Cards are cards based on the Pokémon Trading Card Game.

Water Energy				"Draw tears"	Causes all enemies in the room to periodically take 80% of Isaac's damage and fire a ring of 8 tears every half-second, for 4 seconds. Isaac will also create a ring of 8 tears upon using the card.
Lightning Energy				"Draw petrification"	Causes all enemies in the room to rapidly create Jacob's Ladder lasers that deal 10% of Isaac's damage for 4 seconds.
Fighting Energy				"Draw berserk"	Inflicts all enemies in the room with berserk for 4 seconds.
Psychic Energy				"Draw confusion"	Inflicts all enemies in the room with confusion for 4 seconds.
Colorless Energy				"Draw hemorrhaging"	Inflicts all enemies in the room with hemorrhaging for 4 seconds.
Darkness Energy				"Draw fear"	Applies Curse of Darkness to the room and inflicts all enemies in the room with fear for 4 seconds.
Metal Energy				"Draw bruising"	Inflicts all enemies in the room with a stack of bruising every second, reaching 4 stacks after 3 seconds. All stacks disappear after 4 seconds. Bonus damage taken by bruised enemies will be 25% of Isaac's damage each stack, rather than a flat +1 damage.
Dragon Energy				"Draw energy"	Activates a random Energy Card effect.
Fairy Energy				"Draw charm"	Inflicts all enemies in the room with charm for 4 seconds.

Miscellaneous Cards

Miscellaneous Cards are cards that don't follow any of the other established categories for cards.

SOCCER				"Some people call it football"	Summons a soccer ball that can be knocked around to damage enemies. Damage enemies take depends on the velocity of the football.
FLOORING UPGRADE				"Rollin' out burgundy style"	Gradually fills the entire floor of the room with a random creep type. Based on the color, creep can be sticky, slippery, or deal 2 damage to enemies per tick. Creep starts to disappear after 5 seconds.
AC 3000				"Guaranteed to keep them cool"	Slows all enemies in the room for 20 seconds. Slowed enemies will be frozen when killed.
Calling Card				"666-S8N"	Summons a friendly Psi Hunter.
Defuse				"Bombs be gone"	Converts Bomb Rocks and Troll Bombs into Bomb pickups. Downgrades explosive enemies and certain explosive-related objects into their regular versions.
Download Failure				"Why are all my enemies sliding around?"	Causes all enemies in the room to become glitched. Glitched enemies periodically have their AI stop working, replacing it with that of Poople.
Gift Card				"Let's wrap things up"	Rerolls item pedestals into Mystery Gift.
Grotto Beast				"One rare in every pack!"	Summons a friendly Fiend Folio enemy from a set. Enemy can be "Common", "Rare", "Epic", or "Legendary" rarity. (Higher rarity enemies are typically stronger than lower rarities.) There's a small chance for the summoned monster to be a "shiny" variant. This is indicated by spark particles appearing around the monster and a unique sound effect when it is summoned. Chance for a monster to be shiny is independent of its rarity.
Imp-losion				"Summon friends for damage dealt."	Throws 3 green Pyromancy fireballs in perpendicular directions away from Isaac that summon Hollow Minions on impact. Hollow Minions disappear when the room is cleared. One fireball will always be thrown in the direction Isaac is moving. If he is not moving, it will instead be aimed towards the closest enemy.
Plague of Decay				"Blight bringer"	Adds 5 Rotten Hearts to the health bar. Fills empty Red Heart containers first, then replaces existing Red Hearts.
Pot of Greed				"Draw 2 Cards"	Spawns 2 random cards.
Skip Card				"Nope!"	Clears the room of every grid, pickup and enemy. Rooms cleared using Skip Card do not recharge active items, nor drop room clear rewards. Can skip Boss Rooms at the cost of no item pedestal dropping. Can be used against final bosses to skip phases, excluding the final phase.
Small Contraband				"Deliver it"	Can only be acquired from Fake Beggars, which can drop this item and disappear when interacted with. Does nothing when used, except create a small puddle of white creep. If carried to the next floor, a Dealer will appear in the starting room and exchange it for a random item from the Drug Dealer item pool. Dropping it will have a Snagger spawn from off-screen that will attempt to steal the Small Contraband and fly away with it.

Trivia

• +3 Fireballs was the first and only card to exist in the Afterbirth+ version of the MOD.
• Download Failure's sprite is a reference to missing image placeholder icons used on older web browsers, which was used in place of images that could not load properly.
  • One of Download Failure's voiceovers (as well as its tagline), "Why are all my enemies sliding around?", alludes to a commonly reported glitch in the game by players who have not installed mods such as Fiend Folio correctly, hence Download Failure's name and effect.
• The number "666" in Calling Card's description is the number of the Beast in the Bible, while "S8N" simply alludes to "Satan".
• The friendly Psi Hunter spawned by Calling Card has green eyes instead of red.
• 13 of Stars is, in fact, not a real playing card, instead being a reference to page 6141 of Homestuck.
  • This also ties in with the fact that 13 of Stars can spawn Perfectly Generic Object, another item that is a reference to Homestuck.
• Gift Card was one of the last features added in the Reheated update. Its flavor text "Let's wrap things up" was made as a double-meaning because of this.
• Many of the miscellaneous cards are references to various card games, board games, and series:
  • SOCCER, FLOORING UPGRADE, and AC 3000 reference Card-jitsu from Club Penguin.
  • Defuse is based on the card of the same name from Exploding Kittens.
  • Grotto Beasts is from the card game of the same name made by Twitch streamer Jerma985.
  • Imp-losion is a card in the virtual card game Hearthstone.
  • Plague of Decay is a reference to Summoner Battlegrounds, a card game for Tabletop Simulator made by Fiend Folio developer Cake.
  • Pot of Greed is from the Yu-gi-oh! trading card game.
    • In addition, the "What does it do?" meme that is commonly associated with this card is also referenced.
  • Skip Card appears as the skip card from Phase 10.
• Three of Wands, King of Spades, King of Swords, and 2 of Cups were all added in V2.4.
• Misprinted Two of Clubs was added in V2.5.
• The King of Clubs? card's flavor text and effect are a reference to a scene in Half-Life, where a panicking scientist screams "Oh my God, we're doomed!" and runs into a laser trip mine, resulting in his explosive death.
