Risk's Reward		1	Can be used to reroll an item in the Treasure Room to one of a higher quality. This is achieved by traversing a unique Psionic Cluster and fighting the Hermit.
Torture Cookie		1	Hurts Isaac for a Half Red Heart of health. Either displays an evil fortune, spawns a Black Heart, spawns a random Reversed Major Arcana Card, or spawns an evil-themed trinket.

Clear Case		1	The next active item that is picked up will be assigned to the player's pocket slot.
Black Lantern		1	+1 Black Heart. Guarantees a curse on every floor. Replaces curses with special new types of potentially beneficial curses.
The Devil's Harvest		1	Upon death, respawn as Fiend.
Evil Sticker		1	Spawns 6 Cursed Pennies upon being picked up. Increases the chance for Immoral Hearts, Cursed Pennies, Dire Chests, Spicy Keys, and Copper Bombs to spawn.
Sack of Keys?		1	Spawns a sack familiar that follows Isaac and drops a Spicy Key after every 7 rooms cleared.
Heart of China		1	+1 empty Red Heart container. Overhealing with Red Hearts charges a special health bar. Filling this health bar grants an empty Red Heart container. The number of hearts needed to fill this special bar scales with how many Red Heart containers Isaac has.
Lil Lamb		1	Spawns a familiar that charges to shoot an Ipecac projectile. When Isaac takes damage, has a chance to drop to the floor and target enemies automatically.
Deadly Dose		1	On pickup, spawns a Cyanide Pill. Cyanide Pills provide an all stats up, but will start a death timer that will kill Isaac after 60 seconds. Taking a pill that isn't a Cyanide Pill will cure it, removing the stats and stopping the timer. Increases the chance for Pills to drop on room clear. Pills that drop while Isaac is not under the effect of a Cyanide Pill can be replaced with Cyanide Pills.
Clutch's Curse		1	Familiar that possesses Isaac occasionally. When possessed, the next tear Isaac shoots will be a homing Ipecac tear. Rocks occasionally glow purple and create fire waves upon destruction.
Leftover Takeout		1	+0.3 speed. 1.1× fire rate. 1.5× damage. +1.5 range. +1 luck. Luck-affected chance to fire a fortune cookie tear that deals 5% more damage and display a fortune upon hitting an enemy.
Fist Full of Ash		1	+3 Bombs. More Tinted Rocks spawn on floors, and have odd, more obvious visuals. Breaking Tinted Rocks spawns a Half Red Heart, grants a fading Damage up, and a 50% chance of removing a Broken Heart. Isaac gains a Broken Heart upon entering a new floor.
Birthday Gift		0.5	Replaces all newly encountered collectibles with Mystery Gift. Rerolls and special/fixed drops are not affected.
