Tainted Fiend
- Most of Tainted Fiend's item costumes are the same as regular Fiend but with Black instead of Purple. There are a few exceptions, however.

- The Ugh for Bad Items sprites were created by a member of the Fiend Folio Cornmunity Discord server, GalRosalina.
- Many of the item costumes are easter eggs or references to other Isaac Mods/pop culture:
Fiend
- The statue form of Gnawed Leaf resembles his design from The Devil's Harvest.
Monstro's Lung makes him vaguely resemble Primordial Monstro, a boss from The Devil's Harvest.
Infestation 2 has a 
Baby Spider on his head.
Libra turns half of him into Friend, his rival and a member of Maria's imp gang.
Sack Head makes him appear similar to 
Buck.
2Spooky will give him an appearance similar to 
Flanks.
Mars makes him appear similar to 
Pollution.
Reheated Pizza turn him into The Noise, a character from Pizza Tower.- The Beelzebub transformation turns him into Flysaac, a character from The Devil's Harvest.
- The Spun transformation turns him into a corn cob since canonically he is genetically modified corn.
Golem
Technology is a reference to The Terminator.
My Little Unicorn will give him an appearance somewhat similar to 
Stalagnaught.
SMB Super Fan makes him resemble Tofu Boy, a secret character from the game Super Meat Boy.- Monstro's Lung makes him vaguely resemble Primordial Monstro, a boss from The Devil's Harvest.
- 2Spooky is a reference to the 'becoming uncanny' meme.
Venus is a reference to the Venus de Milo.- The Oh Crap transformation makes him resemble a
Hardy.
Tainted Fiend
Ball of Tar makes him resemble 
Punk.
Delirious makes him somewhat resemble Ash from The End Is Nigh- Reheated Pizza turn him into Fake Peppino, a character from Pizza Tower.
