Bone Worm
|
|
- Burrows out of the ground in a random spot and fires four bones in the cardinal directions, followed by four additional bones in the ordinal directions, then burrows back in.
|
Sternum
|
|
- Crawls around the room randomly.
- When it lines up with Isaac, it will charge towards him until it hits a wall.
- After hitting a wall, it will launch three rib projectiles outwards, then it becomes stunned for a short while.
- After recovering, it will retract the ribs back and resume its normal behavior.

Splodum
|
|
- Crawls around the room randomly.
- When it lines up with Isaac, it will charge towards him until it hits a wall.
- After hitting a wall, it explodes, scattering three arcing bone projectiles around.
|
Doom Fly
|
|
- Bounces around the room.
- Upon death, launches six boomeranging rib projectiles outwards.
|
Crepitus
|
|
- Roams around the room, stopping to launch a volley of arcing bone projectiles towards Isaac if he is nearby.
|
Mr. Bones
|
|
- Roams around the room aimlessly.
- If Isaac draws nearby, it will launch its own head, attempting to bite him, then retract it back.
- If its head is killed before the body, the body will become a Gusher.
|
Possessed
|
|
- Roams around the room aimlessly, firing bones at Isaac if lined in a cardinal direction.
- Upon death, its skull falls to the ground and releases a Moaner.
|
Moaner
|
|
- Chases Isaac for a while, before attempting to return to its respective Possessed's skull, reviving it.
- If there are no skulls to possess, it continually chases Isaac.
|
Cracker
|
|
- Chases Isaac by performing off-screen leaps towards him.
- Each time it lands, it launches four rib boomeranging ribs that travel in a circular pattern.
|
Jawbone
|
|
- Flaps towards Isaac, occasionally making short but quick dashes in his direction.
- Upon death, drops its skull, which can then be revived by Ribbones.
|
Ribbone
|
|
- Slowly flaps towards Isaac.
- If there are any Jawbone skulls in the room, Ribbone will instead move towards them in order to quickly revive them.
|
Ribeye
|
|
- Emerges from pits and fires three boomeranging rib projectiles towards Isaac.
- Ribeye is only vulnerable while attacking.
|
Unpawtunate
|
|
- Remains stationary, occasionally firing explosive bone projectiles at Isaac, marked by a reticle.
- Upon death, its head dislodges and begins to chase Isaac while beeping, exploding after a short period.
|
Fracture
|
|
- Chases Isaac, periodically firing arcing bone projectiles.
- As it takes damage, its head will crack more and more, increasing the amount of projectiles it can fire from 1 to 2 to 3.
|
Molar System
|
|
- Slowly floats towards Isaac.
- As it takes damage, its teeth fall out and begin to orbit it.
- Each consecutive tooth will enter a larger orbit and will also move faster than the previous one.
- When Molar System dies, its tooth orbitals will remain lingering in the air.
|
Lil' Jon
|
|
- Slowly bounces around the room, blocking projectiles and dealing contact damage.
- Dies when the room is cleared.
|
Spinny
|
|
- Bounces around the room, while constantly shooting projectiles in a spiral pattern.
- Takes more knockback and fires faster when being knocked back.
|
Creepterum
|
|
- Bounces around the room, periodically spawning swarms of Attack Flies and Spiders.
- When it dies, its body remains on the ground, dealing contact damage and turning invincible.
- Occasionally, the body will spawn several Attack Flies or Spiders, along with an Eternal Fly for itself, up to three.
- Once it spawns the 3rd fly, Creepterum will resurrect and resume its original behavior.
- Creepterum can only be killed as the last enemy in the room.

Baro
|
|
- Floats around the room, occasionally launching a ball of flies towards Isaac.
- On impact, the ball splits into either three Swarm Flies or three Ring Flies.
- On death, Baro spawns a swarm of Swarm Flies.
|
Ring of Ring Flies
|
|
- Flies towards Isaac.
- On death, splits into eight Ring Flies.
|
Calzone
|
|
- Hops around the room, occasionally spitting out a random Maggot enemy.
- On death, spawns additional maggots.
|
Carrier
|
|
- Crawls around the room.
- When Isaac is in line of sight, it will spit a spider ball at him, which splits into two Spiders on impact.
- On death, spawns Small Crazy Long Legs.
|
Gary
|
|
- Bursts through one of the room's walls either randomly or after the room is cleared.
- Remains stationary, periodically spawning random maggots.
- Retreats upon being defeated and blocks the hole in the wall with rubble.
|
Dangler
|
|
- Slowly floats towards Isaac, periodically firing an irregular spread of three bullets.
- When it damages Isaac, it will steal a couple of coins from him.
|
Tombit
|
|
- Roams around the room, periodically slamming its body into the ground, spawning a Gravin.
- Invincible, dies after the room is cleared.
|
Gravin
|
|
- Remains stationary, does not deal contact damage.
- Doesn't need to be killed in order to clear the room.
- On death, releases four short-ranged rock projectiles in cardinal directions.
