Fiend features a wide variety of unique appearances that he can acquire the different items in The Binding of Isaac: Repentance, Fiend Folio: Reheated, and even other mods. This section features some of the more notable costumes.

  • The Ugh for Bad Items sprites were created by a member of the Fiend Folio Cornmunity Discord server, GalRosalina.
  • Many of the item costumes are easter eggs or references to other Isaac Mods/pop culture:
    • The statue form of Gnawed Leaf.png Gnawed Leaf resembles his design from The Devil's Harvest.
    • Monstro's Lung.png Monstro's Lung makes Fiend vaguely resemble Primordial Monstro, a boss from The Devil's Harvest.
    • Infestation 2.png Infestation 2 has a Baby Spider.png Baby Spider on his head.
    • Libra.png Libra turns half of Fiend into Friend, his rival and a member of Maria's imp gang.
    • Sack Head.png Sack Head makes Fiend appear similar to BuckIcon.png Buck.
    • 2Spooky.png 2Spooky will give Fiend an appearance similar to Flanks.png Flanks.
    • Mars.png Mars makes Fiend appear similar to PollutionIcon.png Pollution.
    • Reheated Pizza Reheated Pizza turns Fiend into The Noise, a character from Pizza Tower.
    • The Beelzebub transformation turns Fiend into Flysaac, a character from The Devil's Harvest.
    • The Spun transformation turns Fiend into a corn cob since canonically he is genetically modified corn.
