Beacon
|
|
- Buzzes around erratically, making quick dashes.
- Occasionally winds back and charges towards Isaac, only stopping and stunning itself if it hits a wall.
- If it comes into contact with Isaac it will stick to him and drain 3 active item charges.
- It can drain charges into the negative.

Dizzy
|
|
- Bounces around the room, while constantly shooting projectiles with a Rubber Cement effect in a spiral pattern.
|
Reaper
|
|
- Floats around the room, periodically spawning spinning scythes in one of the two following patterns:
- Spawns four scythes around itself, that spiral outwards.
- Spawns four scythes in the corners of the room, that move directly at Isaac.

Roy Geralt Dericott II
|
|
- Walks after Isaac, creating a large electric ring around himself.
- After awhile absorbs the ring before firing a massive electric Brimstone laser towards Isaac.
- Rotates the laser to follow Isaac.
- Explodes on death.

Missing Link
|
|
- Moves towards Isaac, shooting blood tears at him cardinally when in line of sight.
- When damaged, will flinch and gain invincibility frames, similar to Isaac.
- Needs to be damaged 5 times in order to be killed, regardless of Isaac's damage stat.
- Often spawns with a high-quality passive collectible from a set list and applies its effect onto itself.

Mag Gaper
|
|
- Relentlessly chases Isaac, breaking any obstacles it runs through.
|
Mag Horf
|
|
- Remains stationary and constantly fires large projectiles towards Isaac.
- These projectiles will burst into 4 smaller projectiles when landing. One of the 4 projectiles will be aimed at Isaac.
- Has an infinite detection range.
|
Mag Clotty
|
|
- Slides around the room randomly but will slide after Isaac if he is in range.
- Periodically fires 4 streams of 4 projectiles in the cardinal directions after each slide.
|
Accursed
|
|
- Slowly moves towards Isaac.
- Occasionally will fire a spread of three spectral shots towards Isaac.
- When damaged, it will spawn a Murmur.
- On death, spawns three additional Murmurs.
|
Buckshot
|
|
- Slowly floats towards Isaac.
- When lined up, it will begin to spit out charging Shot Flies at Isaac, while propelling itself backwards.
- Once it reaches a wall, it will loop back to the other side of the room, charging across the screen two more times until it finally smashes into the wall, releasing a ring of projectiles.
- While charging, it will adjust its path, slightly moving towards Isaac.
- On death, it will perform a final charge towards Isaac, smashing into a wall with high speed and scattering a large amount of blood shots everywhere.

Really Tall Boi
|
|
- Remains stationary.
- When damaged, creates a line of poisonous gas towards Isaac, then summons two Dips and five Shitlings in an arc around itself.
- If there are any Shitlings alive, it will only create gas clouds.
|
