Familiars are small companions, usually generated by passive items, that follow and assist Isaac. Effects of familiars include (but are not limited to) pickup generation, shooting familiars that fire their own tears at enemies, and orbitals that act as projectile shields and/or deal contact damage.
Fiend Folio adds its own menagerie of friends to assist the player!

Temporary Familiars
Minions
Minions, also sometimes known as "Fiendish Friends", are tiny imp-like familiars that chase enemies and deal a percentage of Isaac's damage on contact over time, with the tick rate of the damage scaling with Isaac's fire rate. Very rarely, a Minion can land a critical hit that deals 5× Isaac's damage, with the chance to deal a critical hit scaling with luck.
- If the player has the ability to fly, then all Minions created by the player will sprout wings and fly over obstacles.
Minions are primarily spawned from Immoral Hearts or by Fiend, and there are several types: Immoral Minions and Evil Minions, as well as Hollow variants of both of the former types. When the room is cleared while Minions are active in the room, every Immoral and Evil Minion will disappear and leave a Heart pickup of its original type (with one exception).
- Immoral Minions are purple minions created from Immoral Hearts, and each Immoral Minion deals 20% of the player's damage per tick.
- Immoral Hollow Minions are created from Half Immoral Hearts. They appear to be a melting version of regular Immoral Minions and behave identically, but will turn back into a Half Immoral Heart when the room is cleared.
- Evil Minions are black minions created from Black Hearts only while playing as Fiend, and each Evil Minion deals 40% of the player's damage per tick and has a higher chance to land a critical hit.
- Evil Hollow Minions are created from Half Black Hearts only while playing as Fiend, and they appear to be a melting version of regular Evil Minions and behave identically. However, they will not turn back into a Half Black Heart when the room is cleared, and will instead disappear with a puff of black smoke.
- Entering or teleporting to another room before the room is cleared will cause all Minions to be left behind — returning to the room will have them appear in their pickup form with the exception of Half Black Hearts, from where they were last located.
- Exiting the run and returning will also cause this to happen, though this will also apply to Minions that persist in cleared rooms.

- The Minions created by Implosion and Fiendish Amethyst will have a unique appearance, but otherwise still behave like regular Evil Hollow Minions.
- Note: Fiend Mix can create Minions and Hollow Minions of all other Heart types that turn back into their type of Heart pickup when the room is cleared. However, all of them deal the same damage and have the same chance to land a critical hit as Immoral Minions — with the exception of Black Hearts, which will behave exactly like Evil Minions and Evil Hollow Minions.
- Note: Unlike Minions created from other sources, Minions spawned by Fiend Mix, Soul of Fiend, and the Curse of the Imp effect from Black Lantern will persist between each room entered if spawned inside of a room that is already cleared.
- Minions spawned from the Curse of the Imp effect will persist for the next 3 rooms cleared, while those created by Fiend Mix and Soul of Fiend will only persist until a single room is cleared.
Fragile Bobbies
Fragile Bobbies are familiars that act identically to Brother Bobby, but are able to take damage from enemies and their projectiles, and can be killed after taking enough damage.
They can come in various different shades of blue, and will have a random rate of fire between 0.5 seconds and 0.83 seconds, and random tear damage between 2.8 and 4.2.
There is no cap to the maximum number of Fragile Bobbies allowed on screen, and they will last for the entire run or until killed.
Fragile Bobbies are affected by BFFS!, which will increase their size and double both their damage and their health.

Dips
Fiend Folio adds several new Dips for each type of new Poop that can be spawned by Dirty Mind.
- Beehives create Bee Dips that have flight but greatly reduced health, typically dying in a single hit.
- Trashbags create default Dips, as their interaction with Dirty Mind hasn't been coded as of version 2.7.
- Nests create Spider Dips that attack enemies, slowing them down as well as spawning Blue Spiders when dealing damage.
- Shampoo create friendly Drops that attack enemies by landing on them from above, much like a normal Drop.
- Cursed Poops create Cursed Dips that have a fear aura, causing enemies who come too close to run away.
- Evil Poops create Evil Dips that have a confusion aura, causing enemies to lose track of Isaac and be stunned.
- Platinum Poops create Platinum Dips that martyrize enemies they deal lethal damage to, creating a ghost of them with a holy aura that increases Isaac's tears and damage when inside it, much like White Poop.
- Fuzzy Poops create Fuzzy Dips that spawn a Blue Skuzz on death. This can only be encountered when playing as Tainted ???.
- Obtaining Dad's Dip while having Dirty Mind will create a unique Dip familiar with a green cap, similar to that seen from the Dad's Wallet costume.
The addition of naturally-appearing Petrified Poop and Poison Poop as obstacles allows access to Stone Dips and Poison Dips with Dirty Mind, without needing Tainted ??? or IBS.
Clots
The two new types of hearts added in Fiend Folio, Immoral Hearts and Morbid Hearts, can be consumed by Sumptorium and turned into unique clot familiars.
